This policy covers the consultation request pages Prism Wellness PLLC runs at this address. It explains what we collect when you fill in the form, what we do with it, and how to reach us about it.

What we collect

When you submit the consultation form we collect only what you type into it:

• Your name
• Your email address
• Your phone number
• Anything you choose to write in the message field
• Which service you are seeking
• How you heard about us
• How you plan to pay, which is either self-pay or the name of your insurance carrier
• Which page you sent the request from, so we know what you were reading

We ask for the name of your carrier only. Never a member ID, a policy number or a card number, and you should not send those through this form.

Please do not send clinical details, diagnoses or any other sensitive health information through this form. A short sentence about what you are looking for is all we need. We will take the rest over the phone or in session, where it belongs.

What we do with it

Your request goes straight to our care coordinator so she can call you back, answer your questions and match you with a therapist. That is the only reason we collect it. We do not sell your information, we do not rent it, and we do not share it with anyone outside our practice for marketing.

Analytics and advertising

These pages use Google Ads conversion tracking so we can tell which ads bring people to us. It records that a form was submitted or a phone number was tapped. It does not send Google your name, your email, your phone number or anything you wrote in the message field. You can opt out of Google's advertising cookies through your browser settings or Google's own ad settings.

We also use Google Analytics to see how many people visit these pages, which ads or searches brought them, and which pages they read. It is used for counting and for improving the pages, not for building a profile of you. It does not receive your name, your email, your phone number or the contents of the message field. You can opt out across every site that uses it with Google's browser add on.

We also use Microsoft Clarity to understand how visitors use these pages, such as where they scroll, what they tap and where they get stuck, so we can make the pages clearer. Clarity masks what you type by default, so the contents of the form fields stay hidden in these recordings. You can read more in Microsoft's privacy statement.

How long we keep it

If you become a client, your information moves into our clinical record and is kept under the retention rules that apply to health records in North Carolina. If you do not become a client, we keep the request only as long as we need it to follow up with you, and then we delete it.

How we protect it

The form is submitted over an encrypted connection, and access to the messages it generates is limited to the staff who need it to do their jobs.

Your choices

You can ask us what we hold about you, ask us to correct it, or ask us to delete it. Call us and we will take care of it.

Emergencies

This form is not monitored around the clock and it is not a crisis line. If you are in immediate danger, call or text 988, or call 911.
